Dear Patient,

You’re invited to join the LYNPARZA & Me App, a mobile app designed to empower, educate, connect, and inspire you on your journey with LYNPARZA.

When you download the LYNPARZA & Me App, you’ll be joining a community of Canadians taking LYNPARZA, or those who are caring for someone who is taking this medication. You will be able to set up medication reminders, record any side effects using daily trackers, track upcoming healthcare appointments, record important notes, download a snapshot of your journey in a health report, and access a variety of lifestyle tips – all in the palm of your hand!

To download the app and join the community today, scan the attached QR Code using your phone – detailed instructions are provided below. You will need your LYNPARZA DIN to create your account. The DIN is an eight-digit code found on the medication packaging.

Not sure what a QR Code is? No problem! Here’s how to scan the attached code:

Step 1- Identify the QR Code: A QR code is a small black and white square barcode with a unique pattern of lines and boxes. It’s outlined in yellow and located beside the images of the smartphones in the attached brochure (it says “scan me” underneath!).

Step 2 - Scan the Code: To scan the code, open the camera on your phone and hold it over the QR code on your screen as if you were going to take a picture of it. The code must be clearly visible within your smartphone’s screen.

Step 3 - Click the Link: Your phone might automatically scan the code, or you may have to press a button to snap a picture, similar to the button on your smartphone camera. A link should pop up on your phone screen. Click this link to download the app!
