“Personal Information” as used in this policy means information about an identifiable individual, such as the name and date of birth of that individual, and may also include information that can be utilized along with other non-personal information to identify the individual.
Information Collection and Use
While using our Service, we may ask you to provide us with Personal Information that can be used to contact or identify you, as well as to provide Service. In coordination with other stakeholders in your care (for example: patient support programs, private and public payors, prescribing physicians, and others as deemed necessary), we may share particular elements of your information for offerings aimed at improving your care. This may include, but is not limited to, initiatives aimed at improving adherence and therapeutic outcomes. Any information that is shared by BioScript with third parties will be de-identified in a way that does not link your Personal Information to you. Your information may be compiled with other information and used as part of aggregate, de-identified data. We may use or share with third parties such de-identified, aggregate data.
Log Data and third parties (BioScript Website)
We collect information that your browser sends whenever you visit our Service (“Log Data”). This Log Data may include information such as your computer’s Internet Protocol (“IP”) address, browser type, browser version, the pages of our Service that you visit, the time and date of your visit, the time spent on those pages and other statistics.
In addition, we may use third party services such as Google Analytics that collect, monitor and analyze this type of information in order to increase our Service’s functionality. These third party service providers have their own privacy policies addressing how they use such information.
Cookies are files with small amount of data, which may include an anonymous unique identifier. Cookies are sent to your browser from a web site and stored on your computer’s hard drive.
We may employ third party companies and individuals to facilitate our Service, to provide the Service on our behalf, to perform Service-related services or to assist us in analyzing how our Service is used.

BioScript has implemented various security measures and strives to use commercially acceptable means to protect the Personal Information collected from you. However, no method of transmission over the Internet, or method of electronic storage is 100% secure and thus, we cannot guarantee its absolute security.
Transfer of Your Information Outside of your Province
Your information may be processed and/or stored outside your province of residence. We undertake measures to protect your personal information when it is stored and/or processed in other jurisdictions. If your personal information is transferred outside your province of residence, it may be available to the provincial entity controlling it is situated under a lawful order made in that province and used for purposes other than those described herein. By providing us with your information, you are allowing your personal information to be transferred outside of your province of residence.
Updating and Accessing Your Information’
We encourage users to update the information provided to us, such as providing a new mailing or email address, a name change, or a change in the medical condition(s) that you have notified us about. This will help us continue to provide Services to you that best meets your needs. Upon written request and subject to applicable law, we will inform an individual of the existence, use and disclosure of his or her personal information and we will give the individual access to that information. An individual can challenge the accuracy and completeness of the information and have it amended, as appropriate.
